Job Description

IN-HOME PSW SUPPORT AND CARE for a patient who is paraplegic rendered bed-bound by pressure wound.

When not provided by Home Care:
– Daily in-bed Bath,
– Change of Disposable Garments including peri-area cleaning, applying
topical products as needed such as ointments, and bundling spent garments for
disposal
– Daily Grooming including clothing change between sleepwear and day clothing, hair care, finger nails/toe nails care, and makeup
– Preparation of daily breakfasts and lunches (hot and cold) including clean up and listing of food items needing purchase
– Advance preparation of supper meals if spouse has a prolonged absence or medically incapacitated
– Light house cleaning as may be needed. Weekly change of bedding or sooner as may be
needed
– Daily washing of dishes (by hand and by dishwasher) and placement into kitchen cupboards/drawers
– Bring daily medications and nutritional supplements to the bedside and assist with their timely ingestion
– Manage Hoyer Lift to safely transport patient for occasional periods of sitting at home
– Assist with patient transfer procedures when leaving the home re Hoyer Lift, Garage Lift, and Accessible Van
– Provide support for safe and secure Transport while accompanying person in need of care for medical appointments including Lakeridge Health-Oshawa, St. Michael’s Hospital-Toronto, and The Scarborough Professional Centre
Participate with prescribed physiotherapy exercises at home for voice, arms and
legs and keep records as needed
– Monitor wound care
– Assist the Home Care Nurse in tracking of supplies provided by Home Care to aid in timely reordering by the Home Care Nurse

Adjust days and/or hours of work as occasionally needed to accommodate needed support including overnight stays
